The information in this L.I.P Help Guide is for an enrolled deed poll where you download the government forms, complete them, and send them to the Royal Courts of Justice with a fee. All organisations will accept a deed poll issued by this method.

WHAT IS LOCO19 MINOR DEED POLL PACK USED FOR?

LOCO19 minor is a deed poll pack which contains the forms LOCO22, LOCO23, LOCO24 and LOCO26. The LOCO19 minor deed poll pack is used to make an application to change the name of a child under the age of 18 years. 

  • Form LOCO22 is used to change your child’s name by deed poll.
  • Form LOCO23 is a suggested form of affidavit of best interest. This is mandatory for all applications regarding minors. It must be completed by the parent with parental responsibility and signed in front of solicitor, commissioner of oaths or an officer of the senior court. This affidavit must state the reasons why you think it is in the best interest of the child to change their name.  
  • Form LOCO24 is a statutory declaration for a minor to change their name. It confirms you (the person with the parental responsibility) are who you say you are. Form LOCO24 is a sworn document which declares in writing that they have seen the relevant documents/evidence/exhibits (such as passport) of the child and the person with parental responsibility and they are who they say they are.
  • Form LOCO26 is a notice for the London Gazette for your child’s name change.

Please note – you will need to attach the written consent of all individuals who have parental responsibility that they agree to the child’s name change. If your child is between the ages of 16-18 years then you will need to attach a letter of consent from them stating that they are aware of the name change and they agree with it. If you do not have a consent letter from the other parent or any individual with parental responsibility then you will need to attach a court order stating you can change your child’s name. To get this court order you will need to make an application to your local family court. The form you will need is form C100. You will use form C100 to apply for a specific issues order.

WHAT IS THE FEE TO SEND LOCO19 MINOR DEED POLL PACK TO THE COURT AND HOW TO PAY FOR IT?

The fee to send LOCO19 minor deed poll pack to the Royal Courts of Justice is £48.32. You can make your cheques, postal order, or bankers draft payable to ‘HMCTS’. You can also pay by cash or credit/debit card in person at the court itself or by calling the number 0203 936 8957 (option 1).

HOW TO COMPLETE LOCO19 MINOR DEED POLL PACK  AND WHERE TO SEND IT.

  • NOTE 1 – A deed poll is a legal document that you give to someone, a company, or an organisation so they can change yours or your child’s name on their records.
  • NOTE 2 – Your application for a deed poll will be sent to the Royal Courts of Justice and they will issue the deed poll.
  • NOTE 3 – The applicant is the child whose name is being changed although the forms are being completed by the person with parental responsibility. 
  • NOTE 4 – As stated above, the information in this L.I.P Help Guide is for an enrolled deed poll where you download the LOCO19 minor deed poll pack, complete the forms within, and send them to the Royal Courts of Justice with a fee. A child between the ages of 16 – 18 years can make an unenrolled deed poll themselves but some organisations may not accept this.

  • NOTE 5 – You can complete your LOCO19 minor deed poll pack online and then print hard copies. Alternatively you can download LOCO19 minor deed poll pack, print the pack and then complete the forms by black pen. 
  • NOTE  6 – If at any stage you find there is not enough space to complete your answers on these individual forms then please use additional sheets of paper to continue on and attach them to your forms. Please write your name, address and question number on the top of your additional sheets of paper. 
  • NOTE 7 – Please keep a copy of your completed LOCO19 minor deed poll pack and any attachments for yourself outside of the ones you send.
  • NOTE 8 – LOCO19 minor deed poll pack is comprised of 4 forms – LOCO22, LOCO23, LOCO24 and LOCO26.
  • LOCO19 minor deed poll pack is 10 pages long.
  • FORM LOCO22 – Deed Poll (child form)
    • You will first be asked to write the date in the space provided at the top of the form.
    • You will be asked to write your name (the person with parental responsibility) in the box below or write down the details of the court order giving permission to change the child’s name.
    • You will then be asked to write your child’s NEW name in full.
    • In the box underneath you will be asked to write your child’s OLD name in full.
    • You must now write the age of your child in the box underneath, The form mentions your child is single.
    • You will then be asked your child’s status and citizenship (British citizen, British Dependent Territories citizen, Commonwealth citizen). You must tick the relevant box.  
    • You must now complete the next section and then sign your form LOCO22 in the presence of 2 witnesses who must also sign the form. The witnesses must write their name, address and occupation. 
  • FORM LOCO23 – Suggested form of affidavit of best interest.
    • Please write your full name (the person with parental responsibility) in the first box.
    • Now write your full address in the box below.
    • In the boxes below tick whether you are the mother or father. Then write your child’s full NEW name and the OLD name in the boxes underneath.
    • Now write your child’s age and date of birth. The form mentions your child is single.
    • You must now tick whether you were married to or were in a civil partnership and write the name of the other parent. You must tick the appropriate box whether your child is ‘MY’ or ‘OUR’ child.
    • Please state the reasons why you would like to change your child’s name in the box underneath and attach any evidence that will support your application.
    • As the parent with parent responsibility you must sign the form in front of a solicitor, commissioner of oaths or an officer of the senior court. They must sign give their details and full name in the boxes provided. The other parent can also sign this section if they give permission for the child to change their name.
  • FORM LOCO24 – Statutory declaration for a child
    • This form is to confirm you (the person with parental responsibility) are who you say you are.
    • LOCO24 is completed and signed by a person who is not related to you by blood or marriage, must be a British citizen, must be a resident in the UK, a homeowner, and they must have known you for over 10 years. This person is called the declarant. A solicitor, commissioner of oaths, or a senior court officer must also sign the form as well. They may charge you a fee for this. If you as the parent with parental responsibility do not know anyone for 10 years or over to act as the declarant, you must attach an additional affidavit and explain the reasons why. 
    • The declarant will need to write their full name, address and occupation in the first three boxes at the top of the form. 
    • They will then have to tick the box as to their citizenship.
    • The declarant will then need to write how long they have known you and write your name in the box.
    • The declarant will write your child’s NEW name and OLD name in the relevant boxes and how many years they have known your child.
    • The next section will ask the declarant to declare that you (the person with parental responsibility) have shown them the necessary documents (also called exhibits). The declarant must do this for each document/exhibit (please see ‘What are exhibits and what must be exhibited to the statutory declaration on page 3 of LOCO19 minor deed poll pack). These documents are –
      • Exhibit A is a copy of the deed poll (form LOCO22)
      • Exhibit B is evidence of British citizenship of the child such as a birth certificate/passport/certificate of naturalisation.  
    • The declarant must sign the form in the presence of a solicitor, commissioner for oaths or an officer of the senior court. The solicitor may charge a fee.
  • FORM LOCO26 – Notice for the London Gazette for a child
    • You will need to write the date of the deed poll in the box at the top of the form.
    • You must then write your child’s NEW name.
    • You must then give your child’s status and citizenship. You must tick the relevant box. 
    • You will then be asked to write your child’s old name and new name in the spaces provided.
    • Please date the form and give your solicitor details is you have one. 
  • You cannot send your forms in by email. They have to be sent by post or hand delivered.
