Forms Used In The CARE Process

 

This page lists the 3 most important forms you may need to obtain court orders with regards a looked after child (a child in care).

BOX 1 – Form C1 is the form used to apply to the court for other orders under the Childs Act 1989, except a care and supervision order, a section 8 order, and an enforcement of a child arrangements order. With reference to the care process form C1 is used  to ask the court for more contact with a child in local authority care (a looked after child).

BOX 2 – Form C2 is used to ask permission to start proceedings where permission is required. Ask to join or leave existing proceedings. Request an order or directions within existing proceedings relating to children.

BOX 3 – Form C15 is a supplementary form which is sent alongside form C1 to the court to ask for more contact with a child in local authority care (a looked after child).

Form C1
