Form A65

Form used when you would like to keep your contact details confidential from the other party. This form is usually used alongside form A53 which is used for post adoption contact. The contact details of the applicant (the person making the application) will be omitted on form A53 but will be written on form A65 instead. The court will not disclose any contact details.

 

WHAT IS FORM A65 USED FOR?

Form A65 is used to ask the court to keep your contact details confidential from other parties during court proceedings under the adoption and children act 2002. It can also be used to keep the contact details of the child confidential as well. 

Form A65 is a supplementary form you send to the court alongside other forms such as form A53. So we can explain to you how form A65 is used we will use the example of form A53 in this explanation. Form A53 is the form used to apply for post adoption contact. If you are submitting form A53 to the court, once they receive and process it they will send a court stamped copy to other parties in the proceedings. If you want your contact details to be kept confidential then you leave the contact details section blank on your form A53. Instead you must write your contact details on form A65 which the court will keep. 

If you are submitting an application form to the court you are generally known as the applicant. If you are being asked to reply or acknowledge another’s application then you are generally known as the respondent. If you are the respondent in proceedings and you wish to keep your contact details confidential, when you receive your notice of proceedings please complete your acknowledgement of the notice form and send form A65 alongside it to the court.

WHAT IS THE FEE TO SEND FORM A65 TO THE COURT AND HOW TO PAY FOR IT?

There is no fee to send this form to the court.

HOW TO COMPLETE FORM A65 AND WHERE TO SEND IT. 

  • NOTE 1 – You can complete this form whether you are the applicant or the respondent.
  • NOTE 2 – You can complete form A65 online and then print a hardcopy. Alternatively you can print the form and complete with black pen.
  • NOTE 3 – Please keep a copy of your form A65 for yourself outside the copy you send to the court.
  • NOTE 4 – You only need to send 1 copy of form A65 to the court.
  • NOTE 5 – Please do not attach form A65 to the form you are sending it to the court with. Send it with the form in the envelope. 
  • Form A65 is 4 pages long.
  • Part 1 on page 1 will ask for your details. If your partner is also involved in the application then you can give their details.
    • Title
    • First name
    • Last name
  •  You will also be asked for the child’s name.
  • Part 2 on page 1 will ask for the details you wish to keep confidential.
    •  Address
    • Telephone number
    • The child’s address
    • The title, first name, and full name of the individual who the child lives with.
    • The new surname of the child if the surname is being changed.
  • Page 3 will ask you to sign and date a statement of truth.
