Form used by an adopted person to register their wishes with the
general register office regarding contact with birth relatives
WHAT IS FORM CR Part 1 USED FOR?
Form CR Part 1 is the form used by an adopted individual to register their wishes with the general register office regarding contact with birth relatives. The adopted individual must be over 18 years of age to make this application. The general register office must also have records of your birth or adoption as well.
Please note that if the adopted individual DOES NOT want any contact with their birth family they can register this request.
The general register office is part of her majesty’s passport office.
Once the general register office has received your form CR Part 1 they will write to you.
- If you have asked for contact with all relatives and any birth relative has contacted the general register office and asked for contact with you, you will be given their name and address. It is up to you to contact them if you wish. This relative will be informed that you have been informed about them but they will not be given any of your details.
- If you have asked for contact with only a specific relative such as birth mother and they have contacted the general register office and asked for contact with you, you will be given their name and address. It is up to you to contact them if you wish. This relative will be informed that you have been informed about them but they will not be given any of your details. If there are any other relatives that have contacted the general register office and asked for contact with you, you will be informed about them and given their name and address. They will not be informed of this.
- If you have asked for NO contact with any birth relatives, the general register office will still infirm you about any birth relatives that have contacted them for contact with you. However, the general register office will not inform them that you have asked for no contact.
- If you have used a third party intermediary the general register office will inform them of any relatives that have asked for contact.
- If a relative has used a third party intermediary agency and this intermediary agency contacts the general register office, they will inform this agency of your wish for no contact. This agency may still contact you to ask if your contact wishes have changed.
You can contact the register anytime and change your details or have yourself removed from the register.

WHAT IS THE FEE TO SEND FORM CR Part 1 TO THE COURT AND HOW TO PAY FOR IT?
There is a fee to submit form CR Part 1 to the general register office is £15.00.
- You can send a postal order or cheque made payable to Her Majesty’s Passport Office and attach it to your form CR Part 1.
- If you would like to pay by credit card or debit card you can input your card details on page 3
HOW TO COMPLETE FORM CR Part 1 AND WHERE TO SEND IT
- NOTE 1 – You can download form CR Part t, print it and complete it by black pen;
- NOTE 2 – If you find there is not enough space to complete your answers on your form CR Part 1 then please use additional sheets of paper to continue and attach them to your form. Please write your name, address, and question number on the top of your additional sheets of paper.
- NOTE 3 – Please keep a copy of your form CR Part 1 and any attachments for yourself outside of the ones you send to the family court.
- NOTE 4 – If you are posting your form then please post by signed for recorded delivery so you have proof of postage in case the general register office loses your application.
- NOTE 5 – The address to send your form is Adoption Section Room C202, General Register Office, Trafalgar Road, Southport, Merseyside, PR8 2HH
- Form CR Part 1 is 3 pages long.
- Section 1 on page 1 will ask you to write your current surname, first name and address, contact telephone number, and email address. The form will also ask you if the general register office can contact you by email.
- Section 2 on page 2 is about your intermediary if you have one. This is the person who is helping you or if you are using a their address. If you have one then please write their details otherwise continue to section 3.
- Section 3 on page 2 will ask about your birth information. If you do not know then please contact the general register office so you can get your birth registration details. You will be asked for your birth surname, birth first name, date of birth, gender, place of birth, birth mother’s surname at the time of the birth, birth mother’s first name, birth mother’s maiden name, birth father’s surname and birth father’s first name.
- Section 4 on page 2 will ask about your adoption details. You will be asked to write your adoptive surname, adoptive first name, adoptive mother’s surname, adoptive mother’s first name, adoptive father’s surname, adoptive father’s first name, the year the adoption took place, and the name of the court where it happened.
- Section 5 on page 2 will ask you to date and sign a declaration. You are declaring that you wish your name to be entered in part 1 of the adoption contact register and you wish to contact a relative or do not wish to contact a relative. In section 5.2 if you want contact you must write with whom you would like contact. You can write ‘all relatives’ or a particular relative such as ‘birth mother’. In section 5.3 you must write who you do not want contact with. You can write ‘all relatives’ or write a particular relative such as ‘birth mother’.
