for a female genital mutilation protection order on behalf of a girl (FGMPO)

 

WHAT IS FORM FGM006 USED FOR?

Form FGM006 is the application form used to apply to the family court to ask for permission to apply for a female genital mutilation protection order (FGMPO) on behalf of a girl.

The form used to apply for a FGMPO is form FGM001. Form FGM006 is sent alongside form FGM001 for the court to consider.

WHAT IS THE FEE TO SEND FORM FGM006 TO THE COURT AND HOW TO PAY FOR IT?

There is no fee to send this form to the family court.

HOW TO COMPLETE FORM FGM006 AND WHERE TO SEND IT.

  • NOTE 1 If you are completing and sending form FGM006 to the court then you are known as the applicant. The person the FGMPO is against is known as the respondent.
  • NOTE 2 You can complete form FGM006 online and then print a hard copy. Alternatively you can download it, print it and then complete by black pen. 
  • NOTE 3 If you find there is not enough space to complete your answers on your form FGM006 then please use additional sheets of paper to continue on and attach them to your form. Please write your name, address and question number on the top of your additional sheets of paper. 
  • NOTE 4 Please keep a copy of your form FGM006 and any attachments for yourself outside of the ones you send to the family court.
  • Form FGM006 is 2 pages long.
  • Part 1 will ask you to write your title and full name in the box provided. 
  • Part 2 will ask you the title and the full name of the person to be protected.
  • Part 3 will ask you to explain the reasons for applying on behalf of the person to be protected. You will need to explain your connection with them and what you now about their circumstances with regards to FGM.
  • Part 4 will ask you to sign and date a statement of truth.
  • As stated above you need to send this form alongside form FGM001 (the application form to apply for a FGMPO).
