WHAT IS FORM FGM007 USED FOR?

Form FGM007 is the application form used to apply to the family court to be joined as or cease to be a party to a female genital mutilation protection order (FGMPO).

WHAT IS THE FEE TO SEND FORM FGM007 TO THE COURT AND HOW TO PAY FOR IT?

There is no fee to send this form to the family court.

HOW TO COMPLETE FORM FGM007 AND WHERE TO SEND IT.

Serving court papers on the other party/the respondent means that they must receive a copy of your application form and any other relevant court papers that require them to be informed, respond, comply with directions or attend court. Depending upon the court application you have submitted you can either do this yourself or more than likely the family court will do this for you. In this case with form FGM007 you are NOT required to serve the other party yourself if you are the individual applicant. The court will do this for you. 

  • NOTE 1 If you are completing and sending form FGM007 to the court then you are known as the applicant. The person the FGMPO is against is known as the respondent.
  • NOTE 2 You can complete form FGM007 online and then print a hard copy. Alternatively you can download it, print it and then complete by black pen. 
  • NOTE 3 If you find there is not enough space to complete your answers on your form FGM007 then please use additional sheets of paper to continue on and attach them to your form. Please write your name, address and question number on the top of your additional sheets of paper. 
  • NOTE 4 Please send 3-4 copies of form FGM007 and any attachments to the court. The court will distribute the copies accordingly. One copy is required for the court and one for each party in the proceedings.
  • NOTE 5 Please keep a copy of your form FGM007 and any attachments for yourself outside of the ones you send to the family court.
  • Form FGM007 is 3 pages long.
  • Question 1 will ask you to write your title, full name and date of birth if you are under 18 years of age in the boxes provided. You will then be asked for your address and contact telephone number and again your date of birth if you are under 18 years of age. If you would like the court to keep your contact details confidential from the party these proceedings are against/respondent then you must not write your contact details on this form. You must complete form C8 and send it to the court alongside your form FGM007. You only need to send 1 copy of your form C8.

  • Now you will be asked for your solicitors details if you have one acting for you. Leave blank if you do not.
  • Past 2 will ask you to explain the reasons you are applying on behalf of the person to be protected in the box provided.
  • Part 3 is asking for details of the respondents who need to be served with a copy of this application. You need to write the title, full name, address of all respondents.
  • Part 4 will ask if you or the person to be protected 
    • Need an interpreter at the court hearing and if so which language and dialect.
    • Have a disability where either of you will need special assistance or facilities when you attend court.
    • Need the court to make any special arrangements for safeguarding such as a separate waiting room.
  • Part 5 will ask you to sign and date a statement of truth.
  • As stated above please send 3-4 copies of your completed form FGM007 and all attachments to your local family court so they can distribute accordingly. The court will keep one copy and send one to each respondent and other named individuals in the application including the person to be protected by the order. Please click on the link below to find the address of your local family court. If you are the applicant and the person protected by the order then you must not serve the respondents yourself.
