Forms Used In The Adoption Process

 

This page lists the 13 most important forms you may need to obtain court orders with regards to adopting a child.

BOX 1 – Form A58 is used to make an application for adoption of a child. This form is used for a private and non agency adoption

BOX 2 – Form EX160 is the form used to apply for help with court fees.

BOX 3 – Form A53 is used by an adopted child, their birth siblings, birth parents and adoptive parents to make an application to the family court to consider contact where the family court has not allowed or agreed to contact during the time of the adoption

Form A53 can also be used to apply to the court to restrict or stop contact.

BOX 4 – Form A59 is the form used to apply for an adoption of a child from a country that is a member of the 1993 (29-05-1993) Hague Convention on protection of children and co-operation in respect of intercountry adoption.  

BOX 5 – Form A60 is the form used to apply for an adoption of a child from a country that is NOT a member of the 1993 (29-05-1993) Hague Convention on protection of children and co-operation in respect of intercountry adoption.

BOX 6 – Form C2 is used to ask permission to start proceedings where permission is required. Ask to join or leave existing proceedings. Request an order or directions within existing proceedings relating to children.

An example of the use of form C2 is when an individual other than the child and adoptive parents (birth siblings and birth parents) would like contact with an adopted child. They must submit application form A53 to the court to this. However they must ask the court for permission to apply first. They can do this by completing and submitting form C2. Form C2 can also be submitted alongside form A53 if need be. 

BOX 7 – Form CR Part 1 is used by an adopted individual to register their wishes with the general register office regarding contact with birth relatives.

BOX 8 – Form ACR52 is the form used to apply to the general register office to register an overseas adoption. 

BOX 9 – Form A61 if the form used to apply for a parental responsibility court order prior to adopting a child from abroad

BOX 10 – Form SC4 is the form you would use if you are an employee and would like to apply for paternity leave and pay when you have adopted a child or had a child through surrogacy. 

BOX 11 – Form SC5 is the form you would use if you are an employee and would like to apply for paternity leave and pay when you have adopted a child from abroad.

BOX 12 – Form SC6 is the form you would use if you are an employee and would like to apply for adoption leave and pay when adopting a child from abroad with your spouse, civil partner or partner.

BOX 13 – Form A65 is a the form used when you would like to keep your contact details confidential from the other party. This form is usually used alongside form A53 which is used for post adoption contact. The contact details of the applicant (the person making the application) will be omitted on form A53 but will be written on form A65 instead. The court will not disclose any contact details.
