Forms Used In The Surrogacy Process

 

This webpage lists the 4 most important forms you may need to obtain court orders with regards to the surrogacy process.

Box 1 – Form C51 is the form used by an individual or couple (the intended parents) to make an application for a parental court order giving them parental responsibility and all legal parental rights for a child they have had through surrogacy.

Box 2 – Form A101 is a consent agreement completed and signed by the surrogate parent(s) of a baby giving up the right to the baby so a parental order can be made in favour of the intended parent(s) who will adopt the baby. 

Box 3 – Form FL415 is a statement of service form used to inform the family court how and when you served a copy of your application form and any other relevant court papers required and to whom did you serve these papers. 

Box 4 – Form C52 is called the acknowledgement form and is sent by the court once they receive form C51.

Form C51

Form C51 is the form used by an individual or couple (the intended parents) to make an application for a parental court order giving them parental responsibility and all legal parental rights for a child they have had through surrogacy.

Form A101A

Form A101A is a consent agreement completed and signed by the surrogate parent(s) of a baby giving up the right to the baby so a parental order can be made in favour of the intended parent(s) who will adopt the baby.

Form FL415

Form FL415 is a statement of service form used to inform the family court how and when you served a copy of your application form and any other relevant court papers required and to whom did you serve these papers.

Form C52

Form C52 is called the acknowledgement form and is sent by the court once they receive form C51.
