Forms Used To Obtain Female Genital Mutilation (FGM) Protection Court Orders

Box 1 – Form FL415 is the statement of service form that informs the court how, when and where someone was served (given) court papers.

Box 2 – Form C8 is the form used to inform the court that the individual would like their contact details kept private and not revealed to the other party or anyone else.

Box 3 – Form D89 is the form used to request for personal service by a court bailiff. You are asking a court bailiff to serve (give) your application/court papers to the other party/respondent. 

Box 4 – Form FGM001 is the application form used to apply for a female genital mutilation protection (FGM) court order to stop it from happening.

Box 5 – Form FGM007 is the application form used to apply to the family court to be joined as or cease to be a party to a female genital mutilation protection order (FGMPO).

Box 6 – Form FGM003 is the application form used to apply to the family court to vary, extend or discharge a female genital mutilation protection order (FGMPO).

Box 7 – Form FGM006 is the application form used to apply to the family court to ask for permission to apply for a female genital mutilation protection order (FGMPO) on behalf of a girl.

The form used to apply for a FGMPO is form FGM001. Form FGM006 is sent alongside form FGM001 for the court to consider.  

Box 8 – Form FGM005 is the application form used to apply to the family court for an arrest warrant when there has been non compliance/breach of a female genital mutilation protection order (FGMPO).
